Intake Forms: Once you have scheduled your appointment, please download and complete the new patient intake forms, which can be found below and will be emailed to you. You may email them in advance of your appointment or bring them on the day of your consultation.
Cancellation Policy: For all appointments, the full fee will be charged if cancelled or rescheduled with less than 24 hours' notice.
Office Hours: Our practice is open for sessions by appointment only, Monday through Saturday.

While we do not accept insurance directly, we can provide our NYC clients with a "super-bill" for submission to your provider for reimbursement if your plan covers out-of-network acupuncture and related services performed by a licensed acupuncturist.
