Call the member services number on the back of your health insurance card and ask the representative:
"Does my policy cover acupuncture treatments from an out-of-network acupuncturist?"
If the representative says "yes" , you may ask for the conditions and benefits such as:
- Is there an annual deductible (the amount you must pay out of pocket before you receive reimbursements)? How much as been met?
- Am I limited to specific diagnostic codes (only specific sessions)? If so what are they?
- Co-insurance: The percentage of the allowed charges the policy will reimburse you for. (25%, 50%, 75%)
- Visit limits: The number of visits each year you will be reimbursed for.
Before you hang up be sure to ask if there are forms you need to use, and the address where you send your requests for reimbursement.
